On October 23, 2013, the New Hampshire Long Term Care Foundation (NHLTCF) will hold its annual "Scholarship Dinner and Evening of Entertainment" at the Manchester Country Club in Bedford, NH.On behalf of the Foundation I am asking if your company would be willing to support the Foundation by becoming a sponsor or donor for the Scholarship Dinner and Evening of Entertainment.

The LTCF is a non-profit 501 (c)(3), charitable organization established to promote the care and wellness of New Hampshire’s elderly citizens through the development of individual caregivers who work in the longterm care provider community. The Foundation provides scholarships to individuals currently working in long-term care and seeking to advance their education, usually through enrollment in a degree program. Since its inception in 1995, the LTCF has awarded over 150 scholarships totaling more than $320,000. The types of employees who have received these scholarships have included nursing assistants, nurses, rehabilitative therapists, administrative and activities employees. The scholarships that will be awarded this year are for the 2013-2014 academic year.

Funds for the scholarship awards are raised through silent auction proceeds, event sponsors, and ticket sales. The Foundation continues to succeed in its mission through the kind and generous support of individuals, provider organizations and vendor partners of providers. The LTCF is asking for your support in their efforts to raise funds for scholarships. Please consider becoming a sponsor with a named scholarship recognizing your organization.

SPONSOR LEVELS:

• Major – $7,500
  Recognition as Major Sponsor on invitation to the event, brochure and signage through event
  As a Major Sponsor you will have the honor of awarding a scholarship in your organization’s name
  You will receive ten tickets to the event along with an award recognizing your contribution

• Premier – $5,000
  Recognition as Premier Sponsor in brochure and on signage through event
  As a Premier Sponsor you will have the honor of awarding a scholarship in your organization’s name
  You will receive six tickets to the event along with an award recognizing your contribution

• Patron – $ 2,500
  Recognition as Patron Sponsor in brochure and on signage at registration
  As a Patron Sponsor you will be recognized by the emcee for your contribution
  You will receive two tickets to the event along with an award recognizing your contribution

• Food  – $1,500
  As a Food Sponsor you will have the choice of which course (Hors d’Oeuvres, Buffet or Desert) to sponsor
  Sponsorship will include recognition on food tables and in brochure

• Wine – $1,000
  As a Wine Sponsor you will have the choice of which type of wine (red or white) to sponsor
  Sponsorship will include recognition at the wine station and in brochure

• Gold – $750
  Recognition as a Gold Sponsor in brochure
  Sponsorship will include a verbal recognition during the event

• Silver – $500
  Recognition as a Silver Sponsor in brochure
  Sponsorship will include recognition on one of the attendee tables

• Bronze – $250
  Recognition as a Bronze Sponsor in brochure

• Silent Auction – $150
  A Silent Auction item will be purchased in your name with your company information attached to the item
  Recognition as a Silent Auction Sponsor in brochure

ATTEND THE EVENT:
Individual Ticket – $125 (Individual tickets will be available for purchase.)
Table Purchase – $1,250 (Purchase a table to ensure your group can sit together! Table purchase includes 10 tickets to the event.)
