Do you have a resident, or maybe
two, that could be better served in an alternate setting? Do you
know a resident that has responded so well to living in a
nursing facility that returning home should be an option but it
simply has not happened due to lack of resources? Do you have a
resident that this applies to that does not have a home to go
back too? If any of this makes you think of any of your
residents, you need to attend this program. The program will
give you information on how this project started, how it will
potentially affect your residents and staff and what resources
are available to you to help the residents out.
The New Hampshire Department of Health and Human Services (DHHS)
has been awarded a Money Follows the Person (MFP) grant designed
to better assist people to transition from nursing homes to
community living. The DHHS contracted with the New Hampshire
Independent Case Managers Association (NHICMA) in collaboration
with the New Hampshire Health Care Association (NHHCA) and
Granite State Independent Living (GSIL) to implement certain
portions of the grant.
